Job summary

a2z Physical Therapy in Maryland seeks a Full-Time Home Health Physical Therapist to deliver one-on-one care in patients' homes, focusing on orthopedic and musculoskeletal conditions.

You will perform evaluations, develop treatment plans, provide manual therapy and therapeutic exercise, educate patients, and coordinate with physicians while traveling across the service area. Travel, independent work, and a supportive therapist-owned environment are emphasized.

Qualifications

• Must be willing to travel throughout our service area.
• Current Maryland Physical Therapist license.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education.
• CPR Certification.
• Valid driver’s license
• Automobile Insurance
• Ability to work independently and manage time effectively.
• Reliable transportation and willingness to travel to patients' homes.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred

Responsibilities

• Provide one-on-one physical therapy in patients' homes.
• Perform comprehensive evaluations and reassessments.
• Develop individualized treatment plans.
• Provide manual therapy and therapeutic exercise.
• Educate patients and caregivers.
• Complete documentation in a timely manner.
• Communicate with physicians, nurse practitioners, and case managers.
