Job summary

Palms Home Care in Lakeland, FL is seeking a dedicated Home Health Physical Therapist (PT) to provide in-home rehabilitation, improve mobility, and help patients regain independence after illness, injury, or surgery.

You will conduct initial evaluations, develop individualized treatment plans, administer therapeutic exercises and manual therapy, educate patients and caregivers, document progress, and comply with Medicare guidelines.

Qualifications

• Current Physical Therapist license
• Graduate of accredited PT program
• 1-2 years of experience preferred in Home Health
• Strong communication skills
• Ability to work independently
• Possesses and maintains current CPR certification.
• Valid driver's license and reliable transportation
• Knowledge of OASIS
• Experience with EMR systems such as Kinnser

Responsibilities

• Perform initial evaluations and develop individualized treatment plans
• Provide therapeutic exercises, manual therapy, and mobility training
• Assess patient progress and adjust care plans accordingly
• Educate patients and caregivers on safety and exercises
• Maintain timely and accurate documentation
• Collaborate with interdisciplinary care teams
• Follow home health and Medicare guidelines
