Job summary

LiveWell is seeking a Physical Therapist to evaluate patients in home care, create treatment plans, and supervise therapy services. You will work with the care team, physicians, and therapy assistants to deliver individualized plans that promote recovery and independence in patients' homes.

Qualifications

• Minimum 1 year of physical therapy experience (1+ years in acute or post-acute care preferred).
• Active state Physical Therapist licensure.
• Valid CPR certification.
• Valid Driver’s License.

Responsibilities

• Determine physical therapy treatment plans in consultation with the care team and physicians through initial and ongoing assessments of the patient's condition.
• Provide and complete all proper documentation.
• Administer physical therapy treatments as ordered by a physician. Direct and oversee treatment given by physical therapy assistants.
• Evaluate the effects of physical therapy treatments and the fit of prosthetic and orthotic devices by observing, noting, and evaluating patients’ progress, and recommending adjustments and modifications.
• Complete discharge planning by consulting with the care team and physicians and assure any appropriate continuation of the therapeutic plan post-discharge (e.g., home exercise programs; recommending and / or providing equipment; recommending outpatient follow-up programs).
• Participate in weekly case conferences and effectively communicate about patient care across disciplines involved.
