Job summary

Mission Healthcare is seeking a licensed Physical Therapist for a home health role in Contra Costa County. The position involves evaluating patients, designing individualized treatment plans, and guiding exercises to improve mobility and reduce pain.

The role requires a current PT license, a Master’s or Doctorate in Physical Therapy, and a minimum of one year of PT experience (home health preferred). BLS/CPR certification and a valid driver’s license are required.

Qualifications

• Active Physical Therapist license in practicing state.
• Master's or Doctorate degree in Physical Therapy.
• Minimum of 1 year of experience as a Physical Therapist; home health experience preferred.
• Current BLS, hands-on CPR certification.

Responsibilities

• Evaluate and assess patients' physical conditions and develop individualized treatment plans.
• Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination.
• Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
• Collaborate with an interdisciplinary team to provide holistic, patient-centered care.
• Maintain accurate and timely documentation of patient progress and treatment plans.
