Job summary

Mission Healthcare is seeking a Home Health Physical Therapist to evaluate patients, develop individualized treatment plans, and implement exercises to reduce pain and improve mobility in Concord, CA. You will educate patients and caregivers and collaborate with an interdisciplinary team for holistic care.

Requirements include an active PT license, a Master’s/Doctorate in PT, and at least 1 year of PT experience (home health preferred).

Qualifications

• Active Physical Therapist license in practicing state.
• Master's or Doctorate degree in Physical Therapy.
• Current BLS/CPR certification.
• Valid driver's license and reliable vehicle.
• Minimum of 1 year of PT experience; home health preferred.

Responsibilities

• Evaluate and assess patients' physical conditions and develop individualized treatment plans.
• Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination.
• Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
• Collaborate with an interdisciplinary team to provide holistic, patient-centered care.
• Maintain accurate and timely documentation of patient progress and treatment plans.
