Job summary

The Pennant Group, Inc. is seeking a Physical Therapist to provide contracted or employed physical therapy services across home health settings in Contra Costa County, CA. The role follows physician plans of care and emphasizes functional assessment and progress toward goals.

Qualifications

• Degree in physical therapy approved by an accredited organization (baccalaureate or master's).
• Licensed to practice as a physical therapist in the state, if applicable.
• Two years of experience as a physical therapist; community/home health experience preferred.
• Good verbal and written communication and organizational skills.
• Maintains current CPR certification.

Responsibilities

• Provides physical therapy services to patients according to physician's plan of care.
• Assesses therapeutic/rehabilitative status and contributes to the total plan of care.
• Evaluates home environment and muscle strength, mobility, gait, ROM; directs therapy treatment.
• Instructs patients and families in use and care of therapeutic appliances.
• Documents clinical progress and discharge planning; participates in case conferences.
• Supervises PT Assistants and, if needed, home health aides every two weeks.
