Vetted Solutions is seeking a Full-Time Physical Therapist (PT) to provide home health care in the South/Central Hillsborough County area of Florida. The role focuses on enabling patients to recover from illness or injury at home.

Responsibilities include mobility and function assessments, goal setting, and planning discharge. Collaboration with the healthcare team and OASIS documentation are essential, with CPR certification required and a valid PT license. Home health experience is preferred.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Unrestricted state license to practice as a Physical Therapist.
• Current CPR certification.
• Preferred: strong organization, communication, attention to detail, and time management for home health and OASIS.

Responsibilities

• Conduct initial and ongoing assessments of mobility, strength, and function.
• Deliver therapy services per physician orders, documenting effectively and ensuring compliance with policies.
• Developing and updating detailed plans of care, including progress tracking and discharge planning.
• Collaborating with healthcare team members, educating patients and caregivers, and supervising clinical personnel as applicable.
• Completing OASIS assessments within scope of practice and adhering to regulations and standards.
