DayToDay Health is seeking a Physical Therapist to join our home health care team, helping patients recover in their own homes.

You will conduct assessments, develop and update plans of care, document progress, and collaborate with physicians and nurses to optimize outcomes.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR certification.

Responsibilities

• Conduct initial and ongoing mobility, strength and function assessments.
• Develop and update plans of care per physician orders.
• Deliver therapy services and document patient progress.
• Modify treatment plans and perform discharge planning with patients and caregivers.
• Supervise PTAs and HHAs and complete OASIS assessments where permitted.
• Ensure compliance with policies, standards and state regulations; communicate with the care team.
