Job summary

The Palms Home Care in Sebring, FL seeks a skilled Home Health Physical Therapist to deliver personalized therapies in patients’ homes, focusing on mobility and independence after illness or surgery.

Responsibilities include initial evaluations, treatment planning, therapeutic exercises, progress assessment, patient education, and coordination with care teams, all while adhering to home health and Medicare guidelines.

Qualifications

• Current Physical Therapist license.
• Graduate of accredited PT program.
• 1–2 years of experience preferred in Home Health.
• Strong communication skills.
• Ability to work independently.
• Possesses and maintains current CPR certification.
• Valid driver’s license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.

Responsibilities

• Perform initial evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, manual therapy, and mobility training.
• Assess patient progress and adjust care plans accordingly.
• Educate patients and caregivers on safety and exercises.
• Maintain timely and accurate documentation.
• Collaborate with interdisciplinary care teams.
• Follow home health and Medicare guidelines.
