Job summary

BAYADA Home Health Care is seeking a compassionate Home Health Physical Therapist (PT) to reliably serve patients in Nevada, MO and surrounding area. This is a Full-Time position.

Visit clients in their own homes, perform assessments, and develop care plans with ongoing evaluations. We’ll train you if you have home health experience, and a $30k retention bonus is available for full-time staff.

Qualifications

• Active and current PT license.
• One year of clinical experience preferred.
• Previous home health care experience welcomed, but not required. We will train you!
• Ability to travel locally to cases as assigned

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Implement wound prevention/intervention plans using evidence-based dressings.
• Develop and implement individualized care plans including manual therapy exercises, gait training, balance, and other interventions.
• Continually assess and revise the PT care plan, and participate with nursing in the multidisciplinary care plan.
• Educate clients and family members in rehabilitative care to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
