Job summary

BAYADA Home Health Care seeks a compassionate Home Health Physical Therapist to deliver in-home care in Joplin, MO and surrounding area on a full-time basis. The PT will assess, plan, and implement rehabilitation services for patients in their own homes.

Responsibilities include home visits, diagnostic testing, care plan development, patient education, and detailed documentation using a touchpad tablet, all within a supportive interdisciplinary team.

Qualifications

• Active PT license required for practice.
• Minimum one year of clinical experience preferred in home health would be beneficial.
• Willingness to travel locally to patient homes and manage caseloads.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Develop and implement individualized care plans including manual therapy and gait training.
• Educate patients and families in rehabilitative care to promote health and independence.
• Accurately document observations and interventions using a tablet in a timely manner.
