Job summary

Qualifications

• PT licensure in Virginia required or by hire.
• Degree in Physical Therapy (Bachelors/Master/Doctorate) required.
• 1 year PT experience preferred.

Responsibilities

• Performs evaluations, determines PT diagnoses, develops plan of care, and performs physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability.
• Provides reassessments including OASIS assessments at appropriate time points to determine level of functioning and updates the plan of care with physician input.
• Builds collaborative relationships with a multi-disciplinary team to support patient care needs and participates in team meetings as assigned.
• Supervises Physical Therapy Assistants and Home Health Aides in providing services and completes supervision documentation.
• Provides therapeutic treatments and evaluates equipment needs; prepares clinical and progress notes; plans for discharge.
• Adjusts treatment plan to maximize benefit and meet discharge criteria, using evidence-based guidelines.
