Responsibilities

• Perform evaluations, determine PT diagnoses and plan of care.
• Provide PT procedures to restore function, improve mobility and reduce pain.
• Conduct OASIS assessments and timely reassessments as needed.
• Collaborate with interdisciplinary team and supervise assistants as required.
• Document treatment outcomes and prepare discharge plans.
