Job summary

Riverside Health System is seeking a staff Physical Therapist for home health in Gloucester, VA. The role includes evaluating patients, diagnosing PT needs, and planning care to restore mobility and reduce disability, while adhering to guidelines and quality standards.

The PT will perform OASIS assessments, revise plans with the physician and interdisciplinary team, supervise assistants, and support team training.

Qualifications

• Bachelor's/Master's/Doctoral degree in Physical Therapy required.
• 1 year PT experience preferred.
• Licensed PT in Virginia (VDHP) required; CPR/BLS within 30 days.

Responsibilities

• Performs evaluations, determines PT diagnoses, develops plan of care, and performs PT procedures to aid in restoring function, mobility, and reducing disability.
• Provides reassessments including OASIS assessments at appropriate time points and updates plan of care with the physician and team.
• Builds collaborative relationships with a multidisciplinary team and participates in team meetings.
• Supervises PT Assistants and Home Health Aides and completes supervisory documentation.
• Delivers therapeutic treatments, documents progress notes, plans for discharge, and evaluates equipment needs as applicable.
• Participates in training and supports team members; drives RHS business as needed.
