Careage Home Health in Washington is seeking a Physical Therapist to join our team. This role provides one-on-one home-based care, enabling you to grow your PT skills while making a meaningful impact on patients' lives.

We offer a full-time, travel-friendly position with a competitive hourly rate ($53-73/DOE), no on-call and no weekends, and benefits such as licensure renewal reimbursement, 401k, continuing education at no-cost, and mileage reimbursement.

Qualificações

• Graduated from an APTA-approved physical therapy program.
• Licensed Physical Therapist in Washington.
• Two years of PT experience preferred.
• Home Health experience preferred.
• Detail-oriented with strong time-management.
• Excellent teamwork and critical thinking.
• Valid WA driver's license.

Responsabilidades

• Provide orders and plan of care under supervision of the Director of Clinical Services.
• Collaborate with home care personnel on patient care planning.
• Direct and supervise PT Assistant and Home Health Aide.
• Take initial patient history and perform initial evaluation.
• Consult with physicians on treatment changes.
• Instruct patients and families in home programs.
• Participate in QA and performance improvement activities.
• Complete and submit OASIS assessments and related documentation.
• Use ICD-10 codes appropriately.
