Freedom at Home is seeking a Physical Therapist for home health in Michigan. You will deliver skilled PT services in patients’ homes, develop individualized plans, and support families with exercises and mobility education.

The role offers per-visit compensation, a sign-on bonus, paid mileage, flexible scheduling, and a collaborative leadership team focused on clinician wellbeing and high-quality patient care.

Qualifikationen

• Must be a graduate of an accredited Physical Therapy program.
• Must hold a current Michigan Physical Therapist license.
• Must have CPR certification.
• Minimum of 1 year PT experience.
• Home Health experience is a plus but not required.
• Valid driver’s license and reliable transportation.

Aufgaben

• Evaluate patients in their homes and develop individualized treatment plans.
• Provide skilled physical therapy services.
• Educate patients and caregivers on exercises, safety, and mobility.
• Complete OASIS and clinical documentation in a timely manner.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Monitor patient progress and adjust treatment plans as needed.
• Promote safe, high-quality patient care and excellent outcomes.
