Job summary

Freedom at Home is seeking a compassionate Occupational Therapist to join our home health team in Southeast Michigan. You will evaluate patients in their homes, develop treatment plans, and provide skilled OT services to improve independence and quality of life.

Qualifications

Must be a graduate of an accredited Occupational Therapy program.
Licensed as an Occupational Therapist in Michigan.
Current CPR certification.
Minimum 2 years OT experience preferred.
Home health experience is a plus.
Valid driver’s license and reliable transportation.

Responsibilities

Evaluate patients in their homes and develop individualized treatment plans.
Provide skilled occupational therapy services to improve independence and quality of life.
Assess home safety and recommend adaptive equipment or modifications.
Educate patients and caregivers on therapeutic exercises, self-care techniques, and safe use of assistive devices.
Complete OASIS assessments and clinical documentation accurately and on time.
Collaborate with physicians, nurses, and other healthcare professionals to achieve the best patient outcomes.
Monitor patient progress and adjust treatment plans as appropriate.
Promote safe, compassionate, high-quality patient care.
