Job summary

Caring Hand Home Health Solutions, Inc. is seeking a per diem Physical Therapist in California to deliver home-based therapy. You will evaluate patients, implement plans of care, and educate families, improving mobility and independence.

This role offers a rate of $160-$180 per visit, flexibility to convert to full-time based on performance, and mileage reimbursement. A valid California PT license and CPR certification are required.

Qualifications

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services per physician’s plan of care and California PT Practice Act.
• Educate patients and families on exercises, equipment use, and recovery goals.

Responsibilities

• Collaborate with physicians and care team—report condition changes and participate in case conferences.
• Supervise PT Assistants and oversee Home Health Aide care bi-weekly when PT is the sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
