Job summary

ALC Health Care Services is seeking a PRN Physical Therapist to deliver on-the-road PT services under the Home Health program. You will provide therapy per physician-planned care while maintaining flexibility to cover a broad geographic area.

Responsibilities include evaluating patients, directing treatment, instructing families, and coordinating with the care team. Per-visit pay is offered with supportive admin to schedule visits.

Qualifications

• Requires degree in physical therapy from accredited program.
• Licensed to practice in state.
• Two years of PT experience; home health preferred.
• Strong verbal and written communication.
• Current CPR certification.
• Must have a valid driver’s license and insurance.

Responsibilities

• Provides physical therapy services to patients according to physician's plan of care.
• Assess and evaluate functional status and develop the plan of care.
• Assess muscle strength, mobility, ROM and rehab potential.
• Directs PT treatment and patient/caregiver instruction.
• Determine priority needs for PT and report patient progress.
• Initiates PT program and trains others in PT goals.
• May train patient in prosthetic device use.
• Identify needs for other home health services and refer as needed.
• Prepare and submit clinical/progress summaries.
• Participate in discharge planning.
• Provide PT consultation to home families/caregivers.
• Provide in-service education to nursing organization personnel.
• Participate in peer consultation.
• Supervise PT assistants as per policy and state regulations.
• Instruct and supervise home health aides when therapy is sole skilled service.
