Job summary

ALC Home Health seeks a PRN physical therapist to deliver home health services across a geographic area, working in patients' homes per physician plans of care.

You will assess functional status, design and implement therapy programs, educate patients and families, and participate in discharge planning with flexible scheduling and route planning. Embedded independence and coordination with the Home Health Director are key.

Qualifications

• Requires degree in physical therapy approved by accredited organization.
• Licensed to practice as a physical therapist in the state.
• Two years of relevant home health experience preferred.
• Current CPR certification.

Responsibilities

• Provide PT services per physician plan of care.
• Assess functional status and plan of care development.
• Direct PT treatment and educate patient/family.
• Document progress summaries and discharge planning.
• Supervise PT aides per policy.
