Job summary

Compassus is seeking a Home Health Physical Therapist to deliver one-on-one therapy in patients' homes, promoting independence and safe daily living activities. You’ll assess, plan, and implement rehabilitation programs, coordinate with caregivers, and document progress.

A BS in Physical Therapy, state license, CPR, and at least two years in acute care or rehab are required. Valid driver’s license essential.

Qualifications

• Bachelor of Science in Physical Therapy required.
• Current State License as a Physical Therapist.
• Valid Driver’s License and CPR; TB screen and Hepatitis consent/declination as applicable.
• Two years’ experience as a PT in an acute care or rehabilitation setting.

Responsibilities

• Work one-on-one with patients in their home to provide PT services.
• Assess, plan, and implement rehabilitation programs and document progress.
