Job summary

VitalCaring is seeking a Physical Therapist to provide patient-centered home health care with autonomy and compassion. You’ll evaluate mobility, design individualized plans with the care team, and deliver hands-on therapy to restore independence at patients’ homes.

The role emphasizes collaboration with nurses and physicians, caregiver education, and progress documentation in the EMR. PRN schedule allows flexible hours and ongoing clinical growth.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health experience preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands‑on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner closely with nurses, therapists, and physicians to ensure coordinated care.
• Document patient care and progress accurately and timely within the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
• Provide guidance and education to caregivers and support staff.
