Job summary

Aveanna Healthcare is seeking a Physical Therapist (PT) for home health in Brandon, FL. You will assess, teach, and deliver therapy interventions directed by the physician as outlined in the home health plan of care.

Collaborate with an interdisciplinary team, perform initial and ongoing assessments, and provide written evaluations with goals and outcomes. Flexible scheduling supports work-life balance while maintaining high-quality patient care.

Qualifications

• Requires degree in Physical Therapy and unrestricted state license in the practicing state.
• Proven proficiency in clinical assessments, documentation, and compliance with policies and procedures.
• Current CPR certification and valid driver's license.
• Demonstrated ability to collaborate with interdisciplinary teams.

Responsibilities

• Assess mobility, coordination, strength and function to establish goals.
• Deliver PT services per physician orders and plan of care.
• Document outcomes, update goals, and plan discharge as appropriate.
• Coordinate with health care team and educate patients and caregivers.
