Job summary

FeldCare Connects is seeking a self-motivated Physical Therapist to deliver premier home health care for adults of all ages. The PT will assess patient needs, develop and execute treatment plans, and monitor progress to promote movement and reduce pain.

Candidates must be licensed as a Physical Therapist in the state and have the ability to work with patients of all ages; bilingual skills are a plus. Flexible scheduling and independence are highlighted in our clinician-focused environment.

Qualifications

• Must hold Physical Therapist license and state registration.
• Completion of a PT curriculum approved by The American Physical Therapy Association or related councils.
• Bilingual a plus.
• Ability and enthusiasm in working with all ages.

Responsibilities

• Assess and evaluate patient care needs to promote movement, reduce pain, restore function and prevent disability.
• Determine treatment plans based on assessments and implement interventions to improve patient well-being and monitor progress.
• Maintain accurate and discreet records and communicate effectively with patients and team.
