FeldCare Connects is seeking a self-motivated Physical Therapist to provide high-quality home health care for adults in the Santa Cruz area. The role involves assessing patient needs, creating treatment plans, delivering PT interventions, and monitoring progress to improve mobility, reduce pain, and restore function.

Applicants must hold a California PT license, complete an approved PT curriculum, and be capable of adapting plans as patient needs evolve.

Qualificações

• Ability and enthusiasm in working with all ages
• Physical Therapist license and CA registration
• Curriculum approved by national PT bodies
• Bi-lingual skills are a plus

Responsabilidades

• Assess and evaluate patient care needs in home health.
• Develop and implement treatment plans to reduce pain and restore function.
• Deliver interventions and monitor patient progress.
• Maintain accurate records with discretion and clinical reasoning.
