Job summary

FeldCare Connects seeks a self-motivated Physical Therapist to deliver home-based care for adults in the San Francisco area. The clinician assesses needs, designs a plan, and executes targeted interventions to improve function and reduce pain.

Qualifications

• Licensed Physical Therapist in good standing
• Experience delivering PT in home health setting preferred
• Ability to tailor treatment plans to adults of all ages
• Bilingual a plus is advantageous

Responsibilities

• Assess patients to determine care needs and develop treatment plans
• Deliver interventions to improve movement, reduce pain, restore function and prevent disability
• Monitor patient progress and adapt treatment as needed
• Document assessments and progress with accuracy and discretion
