Job summary

BAYADA Home Health Care, Inc. in Nevada, MO is seeking a compassionate Home Health Physical Therapist (PT) for full-time work delivering care in patients' homes across Nevada, MO and surrounding area.

You will perform 1:1 direct care, conduct mobility and balance assessments, implement individualized therapy plans, and educate patients and families to promote recovery and independence. A $30,000 retention bonus is available for full-time staff; BAYADA offers flexible schedules, 24/7 clinical

Qualifications

Active and current PT license.
One year of clinical experience preferred.
Previous home health care experience welcomed, but not required. We will train you!
Ability to travel locally to cases as assigned

Responsibilities

Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements, such as mobility and range of joints, transfer status, stability, balance, and ambulation.
Develop and implement appropriate individualized care plans, including manual therapies, gait training, and balance interventions.
Continually assess and revise the Physical Therapy care plan, and collaborate with nursing in the multidisciplinary plan.
Educate clients and families in rehabilitative care and activities to promote health and independence.
Accurately document observations, interventions, and evaluations in the client record using a touch pad tablet.
