Adoration Health is seeking a dedicated Home Health Physical Therapist to join our team in Bluefield, WV. This full-time role offers patient-centered, 1:1 care in a home health setting with supportive team collaboration.

Requirements include a Bachelor’s Degree in Physical Therapy, state licensure, CPR, and 2+ years of experience preferred. Sign-on bonus of 20K applies to external candidates; benefits include health, retirement, and more.

Qualifications

• Minimum of Bachelor’s Degree in Physical Therapy from an accredited program.
• New physical therapy graduates may be considered in select markets based on program availability.
• At least 2 years of experience as a physical therapist; hospice/community experience preferred.
• Valid license in the state of practice and CPR certification.

Responsibilities

• Perform PT evaluation and treatment based on physician orders, including acquisition of adaptive equipment.
• Establish measurable goals and treatment plans to move patients toward independence.
• Maintain ongoing communication with the treatment team, physician, patient, and family.
• Maintain thorough and timely documentation for proper billing.
• Supervise PTAs and PT aides per state act.
