Job summary

Adoration Health, an Adoration Home Health partner, is hiring a Physical Therapist for Bluefield, WV, to join our home health team. You will deliver 1:1 patient-centered care, perform PT evaluations and develop plans that move patients toward independence in the home setting.

This full-time role includes a $20,000 sign-on bonus for external candidates and a robust benefits package: medical, dental, vision; HSA/FSA; tuition discounts; 401(k) with company match; mileage reimbursement; generous

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• New physical therapy graduates may be considered in select markets based on program availability.
• At least 2 years of experience as a physical therapist; community/hospice experience preferred.
• Valid license in the state of practice (WV).
• CPR certification current.

Responsibilities

• Perform PT evaluation and treatment of home health patients per physician orders.
• Establish measurable goals and develop treatment plans to maximize independence.
• Maintain communication with the treatment team, physician, patient, and family.
• Maintain thorough documentation for proper billing and care continuity.
• Supervise PTAs and PT aides in direct patient care as per state practice act.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
