Job summary

UnitedHealth Group seeks a Physical Therapist for Home Health to assess patient mobility and endurance, develop therapy plans, and implement interventions to improve well-being. You will work with the care team to monitor progress and adjust plans based on state guidelines.

The role requires licensure, a valid driver’s license with insurance, and CPR certification or ability to obtain within 90 days. A competitive full-time schedule with benefits is offered.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the care team.
• Observes and reports the patient's response to treatment and any changes in condition to the supervising nurse and physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patients, family, and care team regarding plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment, completes progress notes, and submits notes per policy.
• Completes evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
