Job summary

Advanced Home Health and Hospice is seeking a Home Health Physical Therapist (PT) for PRN positions in the Cincinnati area. You will provide skilled PT services in patients' homes, helping restore function and independence.

Travel to patient homes across Cincinnati and surrounding areas with daytime, Monday–Friday scheduling flexibility. The role requires an Ohio PT license, CPR, and at least 1 year in home health or a clinical setting, with per-visit pay and supportive leadership.

Qualifications

• Active Ohio Physical Therapist license is required.
• 1 year of home health or healthcare facility experience preferred.
• Current CPR certification required.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop treatment plans.
• Deliver skilled therapy in patients' homes and monitor progress.
• Educate patients and caregivers on exercises, mobility, safety, and devices.
• Travel to patient homes in Cincinnati and surrounding areas and document care promptly.
• Collaborate with physicians and care team to update plans as needed.
