Job summary

Stern at Home Therapy is seeking a licensed Physical Therapist to provide one-on-one home health care for geriatric patients in and around Avenel, NJ. The role offers a choice between part-time W-2 employment with benefits or a flexible PRN schedule.

You will evaluate, treat, and manage individualized plans of care, collaborating with an established clinical team to deliver high-quality care in patients' homes. Enjoy clinical autonomy and flexible caseload options.

Qualifications

• Active Physical Therapist (PT) license in New Jersey is required.
• Reliable transportation.
• Strong clinical, communication, and time-management skills.
• Ability to work independently in a home-based setting.
• Home health experience is preferred but not required.

Responsibilities

• Evaluate and treat geriatric patients in patients' homes in the Avenel area.
• Perform PT evaluations and develop individualized plans of care.
• Provide one-on-one treatments based on patient needs and goals.
• Manage treatment plans with clinical autonomy.
• Help patients improve mobility, strength, balance, safety, and function.
• Complete accurate and timely clinical documentation.
• Communicate with patients, families, physicians, and the clinical team.
• Coordinate with the administrative team regarding scheduling and logistics.
• Build and manage caseload based on availability.
