Job summary

Stern at Home Therapy is seeking a PRN Physical Therapist (PT) to provide one-on-one home health care in Edison, NJ and surrounding communities. You will evaluate and treat geriatric patients in their homes, with clinical autonomy and support from an administrative team.

Minimum commitment of 5 patients, flexible scheduling, and the option to supplement another position make this role ideal for maintaining a small caseload and choosing when and where you work.

Qualifications

• Active Physical Therapist license in New Jersey.
• Graduate of an accredited Physical Therapy program.
• Reliable transportation required.
• Ability to work independently in a home-based environment.
• Home health or geriatric experience preferred but not required.

Responsibilities

• Evaluate and treat geriatric patients in homes.
• Develop individualized plans of care.
• Provide one-on-one PT treatments at patient homes.
• Educate patients and caregivers on exercises and safety.
• Complete accurate clinical documentation.
• Coordinate referrals, scheduling, and communications with care team.
• Monitor progress and adjust plans as needed.
