Zusammenfassung

Interim Healthcare of Central, VA (Haslup) is seeking a Physical Therapist to provide home-based therapy in Virginia Beach and surrounding areas. You will help patients regain mobility, manage pain and meet personal goals in a care plan tailored to daily life.

Responsibilities include assessments, documentation, and education of patients and families, with 1:1 therapy and flexible assignments to support work-life balance. Virginia PT license and CPR are required.

Qualifikationen

• Graduate of an accredited Physical Therapist program with active VA PT license.
• Current CPR certification.
• Maximum of 2 years of physical therapy experience, ideally in home healthcare.
• Knowledge of state and federal home health regulations.
• Good clinical judgment, interpersonal skills, dexterity and compassion.
• Valid driver’s license and reliable transportation.

Aufgaben

• Provide home-based therapy to patients who have lost mobility, strength and functionality.
• Work as part of a home health team focused on the patient's plan of care and goals.
• Observe movements, conduct assessments, document progress and modify care plan as needed.
• Assist patient with exercised and hands-on therapy to manage pain and increase mobility.
• Educate patients on their plan of care, established goals, exercises and self-care.
• Educate families on the plan of care, therapy goals and safely assisting with exercises.
