Qualifications

• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.

Responsabilités

• Provide patient care and document treatment per physician orders and agency policy.
• Perform patient evaluations and select diagnostic materials.
• Inform physician of assessment results and recommended interventions.
• Develop and document treatment plans with measurable goals.
• Educate and counsel patients for organized treatment.
• Present patient information to physicians and inform the team of status and plans.
• Administer treatments per patient needs and payer guidelines.
• Set objective goals in collaboration with patients and team.
• Meet productivity standards and manage time efficiently.
