Job summary

AuthoraCare Collective is seeking a Physical Therapist (PT) Case Manager for our Home Health department, serving Greensboro, Kernersville, and Winston-Salem. This is a full-time role with Monday–Friday, 8:00am–5:00pm hours.

The PT provides services to restore function, improve mobility, relieve pain, and prevent disabilities. You will manage the therapy plan of treatment in collaboration with the interdisciplinary team, assess patient problems, set goals, and work with physicians to create

Qualifications

• Graduate degree from an accredited Physical Therapy Program (approved by the APTA).
• Current and unrestricted Physical Therapy Licensure.
• Current CPR certification.
• Minimum of one-year physical therapy experience home health or facility preferred.
• Patient management skills including assessment, critical thinking, problem solving, delegation, and interpersonal/communication abilities.

Responsibilities

• Restore function, improve mobility, relieve pain and prevent disability.
• Manage therapy Plan of Treatment for a caseload of Integrated Health patients in collaboration with the IDT.
• Assess patient problems, establish therapy goals and interventions.
• Collaborate with physician to establish a treatment plan using exercises, hands-on therapy, and equipment.
• Establish and maintain therapeutic relationships and provide community resources/education.
• Document goals and outcomes of care; conduct evaluations and treatments for hospice patients if needed.
