Job summary

Adara Home Health is seeking a Physical Therapy Case Manager to deliver and oversee home health services across Cannon Falls, Red Wing, Lake City and nearby areas. You will travel to clients' homes, provide 1:1 care, and update care plans with a focus on quality and cost efficiency.

The role requires MN PT licensure, strong clinical judgment, and the ability to manage schedules and work with a multidisciplinary team. A sign-on bonus may apply with a competitive compensation package.

Qualifications

• Licensed Physical Therapist in Minnesota without license restrictions.
• Negative TB test or equivalent proof.
• Owns dependable vehicle for home visits.
• Ability to travel to multiple client homes daily.
• Pass background checks as required by state and federal rules.
• Experience in home health or clinical facility setting.

Responsibilities

• Assess and plan client care with families and care team.
• Develop and update home health plan of care at least every 60 days.
• Provide direct care and supervise licensed/unlicensed staff as needed.
• Coordinate care with physicians and community resources.
• Ensure accurate documentation and regulatory compliance.
• Educate clients, families, and caregivers on care plans.
