Job summary

UnitedHealth Group is seeking a Physical Therapist to work in Home Health, conducting assessments and developing plans of care to improve patient mobility and function. You will perform interventions, document progress, and collaborate with the health care team to optimize patient outcomes.

The role features a mixed compensation structure including per-visit point pay and hourly pay, with a comprehensive benefits package and growth opportunities within the Optum family of companies.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and assists in the therapy plan of care.
• Provides PT services within the scope of practice and plan of care.
• Observes and reports patient response to treatment to the supervising nurse/physician.
• Treats patients to relieve pain and restore function.
• Instructs patient and family regarding plan of care, limitations, and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes evaluations and develops the PT plan of care per state guidelines.
• Reports outcomes and anticipated frequency of care.
• Provide care without regard to age, race, creed, color, sex, disability, or financial condition.
