Mary Free Bed at Home is hiring a Physical Therapist for a home health role serving Big Rapids and surrounding areas. You will deliver skilled, patient-centered care in patients' homes, perform assessments, and develop individualized therapy plans to improve mobility and independence.

The position includes a first-year $10,000 success bonus and a sign-on bonus of $5,000–$15,000 depending on experience, with potential for ongoing rewards as you grow with the organization.

Qualificações

• Active Physical Therapist, PT license in the state you work
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations

Responsabilidades

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
• Deliver care in a home health setting with autonomy and support from leadership
