Job summary

Mary Free Bed at Home is a nationally recognized home health and hospice provider seeking a Physical Therapist to deliver skilled, patient-centered care in patients’ homes in Big Rapids and surrounding areas. The role offers a $10,000 first-year success bonus plus a $5,000–$15,000 sign-on bonus based on experience, with additional long-term rewards as you grow with us.

Qualifications

• Active Physical Therapist (PT) license in the state you work.
• Minimum of one year of PT experience delivering patient care.
• Valid driver’s license; ability to drive ~20% of workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting.
• Perform comprehensive patient evaluations and develop therapy plans to improve strength, balance, and independence.
• Educate patients and caregivers on exercises, injury prevention, and safe mobility at home.
• Ensure thorough, compliant EMR documentation.
• Travel to patient homes within assigned territory.
• Collaborate with an interdisciplinary team.
