Interim HealthCare of Salina is seeking a dedicated Physical Therapist (PT) to deliver personalized home health therapy, helping patients regain mobility and confidence in their own homes. You’ll assess functional needs, set measurable goals, and provide hands-on treatment in patients’ living environments.

Qualifications

• Graduate of a physical therapy program and licensed to practice in Kansas.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong clinical judgment and documentation skills.

Responsabilités

• Evaluate function, mobility, strength, balance, safety, and equipment needs in the home.
• Help develop the plan of care with measurable short- and long-term goals.
• Provide therapeutic exercise, gait and transfer training, and other interventions per the plan of care.
• Reassess at each visit, complete re-evaluations, and obtain revised orders when indicated.
• Educate patients, families, and caregivers on exercises, fall prevention, and safe mobility.
• Supervise physical therapist assistants as assigned.
• Document treatment, response, and progress accurately and on time, and support discharge planning.
