Interim HealthCare of Salina is seeking a Home Health Physical Therapist to deliver personalized therapy in patients’ homes across Salina, KS. You’ll assess function, develop goals, and provide therapeutic exercises while coordinating with a supportive clinical team.

The role emphasizes autonomy, patient-centered outcomes, and flexible scheduling, with opportunities for growth and continuous education within a trusted home health network.

Qualifications

• Graduate of a physical therapy program with a current Kansas PT license.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.
• Strong clinical judgment, communication, and documentation skills.

Responsibilities

• Evaluate function, mobility, strength, balance, safety, and equipment needs in the home.
• Develop the plan of care with measurable short- and long-term goals.
• Provide therapeutic exercise, gait and transfer training, and other interventions per the plan of care.
• Reassess at each visit and obtain revised orders when indicated.
• Educate patients, families, and caregivers on exercises, fall prevention, and safe mobility.
• Supervise physical therapist assistants as assigned.
• Document treatment, response, and progress accurately and on time, and support discharge planning.

What You'll Do
