Qualifications

• Bachelor’s or Master’s in Physical Therapy from an accredited college.
• Licensed Physical Therapist issued by the state where you will work.
• Minimum of one year healthcare experience.
• Valid driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Experience with tablets, mobile devices and EMR software.

Responsibilities

• Deliver and manage patient care plans in home health.
• Collaborate with team to ensure quality patient outcomes.
• Maintain timely charting and documentation according to deadlines.
