Responsibilities include creating and updating plans of care, documenting progress, and collaborating with physicians, caregivers, and the health team to optimize outcomes. Experience with OASIS is preferred and driving to patient homes is required.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Unrestricted state license.
• CPR certification, driver's license, and auto insurance.

Responsabilités

• Assess mobility, strength, and function; develop and document personalized care plans with goals and outcomes.
• Deliver therapy per physician orders and document plans of care.
• Collaborate with healthcare teams, patients, and caregivers; supervise PTA, OTA, and HHA.
• Complete OASIS assessments where permitted and support discharge planning with home exercises education.
