Job summary

BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist for Brattleboro, Vermont. You will provide one-on-one care in clients’ homes, develop individualized treatment plans, and coordinate with a multidisciplinary team to deliver rehabilitative services.

The role offers a strong benefits package, opportunities for development, and a retention bonus program for full-time staff. Must hold an active PT license in Vermont and have at least one year of recent PT experience.

Qualifications

• Active PT license in Vermont.
• Minimum one year of PT work experience.
• Graduation from a program approved by the APTA or its accrediting body.
• Strong computer and documentation skills with EMR familiarity.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, balance) and assess safety.
• Develop and implement individualized care plans including exercises and gait training.
• Continually assess and revise the PT care plan and educate clients and families.
