Job summary

Orion Placement is seeking a licensed Physical Therapist to provide in-home services across Concord, CA, in a hybrid field-based role. You will assess mobility, balance, and functional needs to improve independence.

The position offers $75–$90 per hour with a $15,000 sign-on or relocation bonus. California PT license required; home health experience and OASIS/HCHB experience are a plus. You will work with a collaborative team delivering patient-centered care.

Qualifications

• Must hold an active California Physical Therapist license.
• Home health experience preferred.
• OASIS and HCHB/Homecare Homebase experience preferred.
• BLS/CPR certification preferred.
• Field-based or in-home patient care experience is a plus.

Responsibilities

• Provide physical therapy services to patients in their homes.
• Evaluate patient mobility, strength, balance, and functional needs.
• Develop individualized treatment plans to improve independence and quality of life.
• Deliver therapeutic exercises and interventions to improve mobility and function.
• Educate patients and caregivers on exercises, safety, and progress strategies.
• Monitor outcomes and adjust treatment plans as appropriate.
• Collaborate with other healthcare professionals involved in care.
• Complete accurate and timely clinical documentation.
• Travel throughout the assigned territory to provide patient care.

Location: Concord, CA - Hybrid. This is a field-based home health position serving patients in their homes throughout the surrounding territory, with documentation typically completed in the field.

Note: Candidates must have an active California Physical Therapist (PT) license. Home health experience, HCHB/Homecare Homebase, OASIS experience, BLS/CPR certification, and prior field-based patient care experience are preferred but not required.
