Job summary

BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist to perform in-home visits in Randallstown, Maryland. This per visit role covers Reisterstown, Owings Mills, Randallstown, and Pikesville, requiring Maryland licensure and home health experience.

The position offers a per visit schedule, with a $65+ per visit pay and a $10,000 sign-on bonus for full-time employment. The candidate will work as part of a multidisciplinary team to deliver rehabilitative care in clients’ homes

Qualifications

• Maryland PT license required.
• Minimum 1 year PT experience.
• Graduation from APTA-approved program.
• Ability to work independently.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated territories.
• Perform mobility and function assessments.
• Develop and revise individualized care plans with PT interventions.
• Educate clients and families on rehabilitative care and safety.
• Document observations and care provided using EMR.
• Collaborate with nursing in multidisciplinary teams.
