Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist for home health visits in the Boyertown area, PA. The role offers part-time or full-time work schedules and a generous sign-on bonus for full-time employment.

You will perform home visits, develop care plans, and coordinate with a multidisciplinary team to help adult and geriatric clients stay safely at home. New grads welcome to apply.

Qualifications

• Active PA PT license required.
• Graduation from an APTA-approved PT program or CMA/ACAE accreditation indicated by transcript or diploma.
• Ability to work independently with strong time management.
• Experience in home health/visits preferred.
• Proficiency with EMR is preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assess mobility, gait, balance, strength, and safety.
• Develop and implement individualized care plans including exercises and gait training.
• Revise care plans and coordinate with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care and activities for health and independence.
• Document observations and interventions in an EMR/tablet system.
