BAYADA Home Health Care in Cressona, PA is seeking a Home Health Physical Therapist to conduct in-home visits for adult and geriatric clients. New grads welcome; job offers per visit compensation and a sign-on bonus for full-time employment.

The role supports diverse home health services across Schuylkill County. The PT will assess mobility, develop individualized care plans, train families, and document care using EMR on a tablet.

Qualifications

• A current PA Physical Therapist license.
• Graduation from an APTA-approved program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilités

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements of mobility and gait.
• Develop and implement individualized care plans and interventions.
• Continuously assess and revise the care plan with the team.
• Educate clients and families on rehabilitative care and activities.
• Document observations and evaluations in the EMR using a tablet.
