Job summary

Baptist Health Home Care KY, LLC is seeking a Licensed Physical Therapist to provide home-based care to patients under physician orders. You’ll join an interdisciplinary team focused on compassionate outcomes and educating patients and families throughout recovery.

The role offers a guaranteed base salary with an uncapped bonus structure, mileage reimbursement, and a comprehensive benefits package including medical/dental/vision.

Qualifications

• Licensed Physical Therapist in the state of employment.
• Minimum of one year of PT experience in an acute care setting.
• Home care experience is preferred.
• Strong communication with patients, families, physicians, and staff.
• Knowledgeable about therapy best practices.
• Ability to make appropriate PT judgments.
• Detail-oriented and timely.
• Valid driver’s license and auto insurance.

Responsibilities

• Provide skilled PT in a patient’s home as ordered by physician.
• Document and report patient progress to physician.
• Perform OASIS assessments and develop individualized plans of care.
• Complete initial evaluation within 48–72 hours of referral.
• Develop and adjust plan of care.
• Promote continuity of care during admissions, transfers, and discharges.
• Educate patient and family on disease/injury and plan of care.
• Instruct on proper use of equipment.
• Ensure documentation meets Medicare guidelines.
• Supervise PTAs and nurse aides when appropriate.
• Maintain active PT license and communicate changes as needed.
